How to use did you ever?
Use did you ever to ask if something happened during a time period that ended (e.g. in the childhood of a now adult).
To ask a general question in the past simple, start with did, then place the subject and the infinitive without to.
For example, a phrase like this:
Did you ever fall off your bike when you were a kid?
